Oral Sodium Phosphate Products
OSP products include the prescription brands Visicol and OsmoPrep, as well as the OTC Fleet Phospho-soda EZ-prep, which is sometimes used as a laxative. Since 2006, the FDA has continued to receive reports of acute phosphate nephropathy with both prescription and products available in the OTC setting. OSPs & Acute Phosphate Nephropathy
Acute phosphate nephropathy is a rare form of kidney injury associated with the use of OSPs that is characterized by deposits of calcium-phosphate crystals in the renal tubules. Those having an increased risk of acute phosphate nephropathy following the use of OSPs include individuals who:
- are over age 55
- are hypovolemic
- have decreased intravascular volume
- have baseline kidney disease, bowel obstruction, or active colitis
- are using medications that affect renal perfusion or function (such as diuretics, ACE inhibitors, angiotensin receptor blockers, and possibly nonsteroidal anti-inflammatory drugs NSAIDs).
FDA Warning on Visicol / OsmoPrep
In December 2008, the FDA announced that Visicol and OsmoPrep would have a ‘Boxed Warning’ – the administration’s strictest – included in their labeling, and to ensure that the benefits of the drugs will outweigh the risk of acute phosphate nephropathy. The FDA also required the manufacturers to conduct a postmarketing clinical trial to further assess the risk of acute kidney injury with use of these drugs.
